Bio-Identical Hormone Therapy

Testosterone is an important hormone in the male body, produced mainly in the testes, which helps regulate a wide variety of bodily function, from red blood cell production to muscle mass, bone density, fat distribution and sex drive. Low levels of testosterone, known as hypogonadism, can have a wide range of effects on a man's health. The latest research is discovering more and more connections between overall men's health and testosterone levels. Some very major illnesses and health concerns have been linked to low testosterone, including:

  • anemia
  • cardiovascular problems
  • obesity
  • Diabetes

While low testosterone is not the known cause of these illnesses, the discovered links between them suggest that regulating and improving testosterone levels can also help treat these associated issues.

Common symptoms of low testosterone include:

  • Decreased strength and/or muscle mass
  • Frequent fatigue or general lack of energy
  • Poor or declining sleep
  • Irritability and/or depression
  • Inability to concentrate
  • Declining libido
  • Weak erections

Treatment is available for low testosterone. Using an injectable testosterone cypionate, our office can provide Testosterone Replacement Therapy, or TRT. Testosterone Replacement Therapy has been shown to have a number of positive effects on men suffering from low testosterone, including improving strength and muscle mass, boosting energy and improving sex drive and cognitive functions.

If you undergo Testosterone Replacement Therapy, you will have a series of injections, typically every 7-10 days, usually injected into the gluteal muscle. Depending on many factors, including the severity of your condition, these injections can potentially continue indefinitely. Testosterone Replacement Therapy is meant to supplement your lack of testosterone: it does not stimulate your body's own testosterone production. Side effects of TRT are minor, but may include decreased testicular size, fluid retention, liver toxicity, increased red blood cells and acne. If you are already experiencing sleep apnea, TRT may also make your sleep apnea worse.

Counseling Therapy

  • Cognitive Behavioral Therapy (CBT)
    Cognitive behavioral therapy (CBT) is a form of treatment that focuses on examining the relationships between thoughts, feelings and behaviors. By exploring patterns of thinking that lead to self-destructive actions and the beliefs that direct these thoughts, people with mental illness can modify their patterns of thinking to improve coping. Here at The C&G Group, our highly qualified therapist and the patient will actively work together to help the patient recover from their mental illness. Therapy is problem-focused and goal-directed. Because CBT is an active intervention, one can also expect to do homework or practice outside of sessions. CBT has been found to be useful for a wide variety of mental illnesses, including mood disorders, anxiety disorders, personality disorders, eating disorders, substance abuse disorders sleep disorders and psychotic disorders. Studies have shown that CBT actually changes brain activity in people with mental illnesses who receive this treatment, suggesting that the brain is actually improving its functioning as a result of engaging in this form of therapy.
  • Classical Homeopathy, Botanical Medicine & Nutrition
    In classical homeopathy and botanical medicine, the individual is considered as a whole and symptoms from the body, mind and spirit are considered when making recommendation of a particular remedy. Also evaluated are a person's environment and lifestyle. Classical homeopathy most closely matches the totality of the individual's symptoms and personality with the homeopathic remedy in order to stimulate the body's own natural healing response. Homeopathic remedies have been approved by the FDA since 1939, and botanical medicine is one of the fastest growing areas of complementary healthcare.
  • Clinical Hypnotherapy
    Clinical hypnosis is an altered state of awareness, perception or consciousness that is used by specially trained and highly qualified mental health providers for treating a psychological or physical problem. It is a highly relaxed state.

    Hypnosis is a state of inner absorption, concentration and focused attention. Hypnosis is often used as an integrated psychological therapy employing clinical hypnosis and cognitive behavioral therapy. The use of CBT in conjunction with hypnotherapy may result in greater treatment effectiveness. One notable meta-analysis concluded "a 70% greater improvement" for patients undergoing an integrated treatment regimen.
  • Dialectical Behavioral Therapy (DBT)
    Dialectal behavior therapy (DBT) is a modification of traditional cognitive behavioral therapy (CBT) for the treatment of chronically suicidal and self-injurious individuals. DBT differs from traditional CBT in its emphasis on validation – a powerful tool whereby the therapist and the patient work on "accepting" uncomfortable thoughts, feelings and behaviors rather than struggling with them. Once an identified thought, emotion or behavior has been validated, the process of change no longer appears impossible, and the goals of gradual transformation become reality. The term dialectics refers to the therapist's goal of establishing a balance between acceptance and change and effectively integrating these two fundamental principles of successful therapy. DBT also focuses on the development of coping skills – specific behavioral techniques used to combat the disabling symptoms of mental illness.

    As part of the skills-based element of DBT, emphasis is often placed on the development of mindfulness practice and other relaxation techniques. Mindfulness teaches individuals to become increasingly conscious of their thoughts and feelings through observation of bodily sensations. Through this practice, an individual develops the ability to accept distressing thoughts without self-criticism and to tolerate self-destructive urges without acting upon them.
  • Eye Movement Desensitization Reprocessing (EMDR)
    Eye Movement Desensitization and Reprocessing (EMDR) is a comprehensive, integrative psychotherapy approach that has been extensively researched and proven effective for the treatment of trauma. EMDR is a set of standardized protocols that incorporates elements from many different treatment approaches. Complete treatment of the client's targeted issue involves a three-pronged protocol (1-past memories, 2-present disturbance, 3-future actions), that is needed to alleviate the client's symptoms and assure that the problem has been addressed fully in treatment. At The C&G Group, the goal of EMDR therapy is to process completely the experiences that are causing problems, as well as related ones that may be uncovered during the procedure in order to achieve full health.
  • Marital & Relationship Counseling
    The challenges that relationships face today are staggering: finances, employment, children, extended families and more. Research has shown that it usually takes six years from the first sign of trouble in a relationship to the commencement of relationship therapy. While help is available at any stage of the relationship, it is wise to intervene early. It's no secret that today nearly 50% of marriages end in divorce. Add to this the number of couples that find themselves in a state of "emotional divorce." With emotional divorce, you and your partner are still together – technically – but emotionally you couldn't be more separate. With relationship counseling at The C&G Group, once you identify the core and specific problems in your marital relationship, and also identify clearly what you want your marriage to be, it is often very possible to clearly outline how to achieve goals and relationship health.
  • Sex Therapy; Male & Female Sexual Dysfunction
    Sexual issues are a result of negative feelings or traumatic experiences that prevent a person from being able to fully participate during sexual intimacy. Sexual problems can also surface as a product of another mental health issue, such as depression or anxiety, or they may be derived from physical conditions, such as bowel or bladder issues, chemical imbalances resulting from certain medications or changing hormonal levels resulting from menopause or childbirth. Sex therapy works to address questions or concerns surrounding a person's sexuality, and our therapist is trained in helping clients to restore or enhance their enjoyment of sex.

    Developed by Tapas Fleming in 1993, TAT is a technique that can help the body process the emotions, leaving your conscious experience freer. It is a combination of placing attention on a set of statements and applying pressure to specific acupuncture points on your face and at the back of your head.

    It is said to be especially effective for ending stress, especially traumatic stress; releasing limiting beliefs; freeing a person of allergic reactions; gaining self-confidence and a positive outlook on life; relaxation and better health.
  • Trauma & Post-Traumatic Stress Disorder
    When in danger, it's natural to feel afraid. This fear triggers many split-second changes in the body to prepare to defend against the danger or to avoid it. This "fight-or-flight" response is a healthy reaction meant to protect a person from harm. But in post-traumatic stress disorder (PTSD), this reaction is changed or damaged. People who have PTSD may feel stressed or frightened even when they're no longer in danger. Using a variety of therapeutic techniques, our providers work with patients to minimize this response and enable them to react more appropriately and comfortably to triggering encounters in everyday life.

Libido & Sexual Function

Impotence, also referred to as erectile dysfunction, is the incapability to sustain an erection long enough to have sex. While occasional problems keeping an erection may not be a cause for concern, continual trouble with erectile dysfunction may be a sign of an underlying health condition such as heart disease or diabetes. It may also increase a person's stress level, decrease self-confidence and cause problems in personal relationships.

Symptoms of impotence include:

  • Difficulty having an erection
  • Difficulty maintaining an erection
  • Decreased sexual desire

Because male sexual arousal involves the brain, nerves, muscles, blood vessels, hormones and emotions, impotence may be resulting from a problem with any of these areas. Physical conditions, such as heart disease, high blood pressure, Diabetes, Parkinson's disease and low testosterone, are just a few causes of impotence. Some psychological conditions that may cause impotence include: elevated stress levels, fatigue, depression and relationship problems. Other risk factors include: alcoholism and drug and tobacco use.

Treatment options vary depending on the cause of impotence, but may include oral medications, surgery, implants and psychological counseling. There are certain lifestyle changes that one can make to help reduce the signs of impotence, such has quitting smoking, exercising regularly, losing weight, seeking alcohol and drug treatment and working on relationship issues.

Dr. Chorowski is a board certified gynecologist with special interest and fellowship training in behavioral gynecology, human sexuality, and consultation liaison psychiatry.  He is well versed in psychotherapeutic and pharmacologic approaches to treating a variety of clinical concerns, including ADD/ADHD, Depression, Anxiety, PTSD, OCD, Panic Disorder, PMS/PMDD, Sexual function concerns including premature ejaculation and low libido, and more. The depth and breadth of his experience lends itself to a unique perspective, enabling him to work with patients on an individual basis, using technologies such as pharmacogenics to craft a personalized assessment and treatment regimen.

Male Menopause/Andropause

Men, like women, can suffer from the effects of changing hormones.  Doctors say that men receiving hormone therapy have reported relief of some of the symptoms associated with so-called male menopause.

Because there is not a defined period referred to as menopause for men, doctors refer to it as androgen (testosterone) decline in the aging male -- or what some people call low testosterone. While aging men do experience a decline in the production of the male hormone testosterone, the decline in production can also occur with certain medical conditions like diabetes.

There can be a lot of concern about male menopause, and it can cause uncomfortable symptoms. But there are many ways to treat symptoms and stay active and strong. Common symptoms include fatigue, weakness, depression, sleep changes and libido changes. Our providers are familiar with a wide range of treatments for these and more symptoms, and we are happy to discuss therapies such as antidepressants, hormonal medications like testosterone, exercise and diet changes, and more.

Premature Ejaculation

Premature ejaculation is ejaculation that happens just after penetration and before the person wishes. This can cause both partners to be dissatisfied, which in turn leads to anxiety and tension that can make the condition worse. Many conditions can cause premature ejaculation, including anxiety and depression. Premature ejaculation may not require treatment, but if necessary, your doctor can suggest lifestyle changes, medicines and techniques to help treat the problem.
